Being out of drug treatment (thankfully)
for the past two years I do not have access to clinical data but rely on my
direct experience as administrator and clinic director for some 20 years.
You are correct that in the vast majority
of cases one should see no detectable levels of THC metabolite (under 50 ng/ml)
after six weeks. Yet, the medical cannabis patient and the chronic cannabis user
represent a different case. Years of regular cannabis use saturate fatty tissues
with cannabinoids that are but slowly released depending upon metabolic rate,
weight loss, physical activity, etc. How Do I Pass A
Drug Test
Many of our patients were referred by the
criminal justice system. These patients faced legal sanction should they
continue to test positive for illicit drugs. For this reason I directly
supervised our drug testing program at the I-ADARP clinics (Interagency Drug
Abuse Recovery Programs). We insisted that tests were truly random, that staff
was adequately trained, that a chain of custody was established, that all tests
were directly supervised by staff, and that a good diagnostic laboratory was
utilized.
Our initial screens were immunoassay
providing a qualitative result. All positives at our clinics were then retested
by GC/MS both to confirm results and to provide a quantitative measure. It is
from the review of thousands of these tests that I can state that long term
regular users of large amounts of cannabis can easily test positive at a level
of 25-50 ng/ml for 6-12 weeks.
The vast majority of patients seeking
treatment for chemical dependency present with opiates, cocaine, alcohol, and/or
amphetamine as their primary problems. These drugs most certainly should clear
within just a week or so. Most of these patients have also used cannabis but
less frequently than their primary drug. These occasional cannabis users should
also test negative for cannabis within 30 days or so.
The problem arises from the true cannabis
"devotee" who may have experienced daily use of cannabis over a period of years.
These patients will eliminate but only gradually. I would examine the
quantitative levels of a series of urinalysis tests over the course of a month.
With very few exceptions an elimination curve could be detected in all of the
patients who refrained from any further cannabis use but it was not unusual for
these patients to still test positive at 30 days at the level of 25-50 ng/ml.
Clinic personnel and others should not
ignore patient/staff interaction and professional evaluation of behavior. The
complete reliance on qualitative urinalysis to the exclusion of observable
behavior, and reliance on arbitrary time frames for elimination assigned to all
patients regardless of history is simply a mistake and indeed not only unfair
but counter productive. How Do I Pass A Drug Test Patients know that cannabis is the drug most likely to
be identified in urinalysis over time so they often switch to far more dangerous
drugs like the opiates or alcohol which clear in 2-4 days. We are, after all,
comparing dangerous drugs with 2-4 hour half-lives to relatively benign cannabis
with a half-life of 6 days.
